If completing this form, please note that we will expect you to remain available for the date(s) you have said you are able to do. If for any reason you are unable to assist with verification and/or attend the interviews, it will be your responsibility to find a deputy to replace your role.
Please note that this form you will be completing for verification and/or interviews. If you are unable to interview in the interview window please click on the first day on the NOT ABLE TO ASSIST FOR INTERVIEWS slot. (Only consultants can complete the verification process, final years are prohibited from taking part in verification)
The verification and interviews will be done virtually and we will send you all the links and documents you require prior to these events.
Please note recruitment office hours are Monday - Friday 08.00 - 17.00, so if your availability falls outside this window, please ensure you are able to access all required portals.
